Gum disease — the clinical term is periodontitis, with an early stage called gingivitis — is the leading cause of tooth loss in adults. It is an infection of the gum and bone tissue that holds teeth in place. Unlike a cavity, it is painless in most cases until it has progressed considerably, which is why so many people are surprised by the diagnosis.
The infection is driven by bacterial plaque accumulating below the gumline, where a toothbrush cannot reach. The body's immune response to that bacteria causes the chronic inflammation that gradually destroys the ligament and bone anchoring the teeth. As the supporting structure breaks down, teeth become loose and eventually cannot be held in place.
Research has consistently associated periodontitis with systemic health conditions including type 2 diabetes, cardiovascular disease, and adverse pregnancy outcomes. The connection with diabetes is particularly well documented — poorly controlled blood sugar makes gum disease harder to manage, and active gum disease can make blood sugar harder to control. Treating gum disease is therefore not just about protecting teeth; for some patients it is relevant to overall health management.
The earlier gum disease is treated, the more straightforward the treatment. Gingivitis — the reversible early stage — responds completely to a professional cleaning and improved home care. Periodontitis requires deeper treatment, but non-surgical scaling and root planing is effective for most patients who have not lost severe amounts of bone. Waiting, on the other hand, allows the infection to progress to the point where referral for surgical care may be the only remaining option.
We measure the depth of the space between the gum and tooth at several points around every tooth using a small probe. Measurements of 4 mm or more, along with bleeding on probing, indicate disease. We also take bone-level X-rays to assess how much supporting bone remains. Together, these give us an accurate picture of where you are on the gingivitis-to-periodontitis spectrum and what treatment is appropriate.

For periodontitis, the foundational treatment is scaling and root planing — often referred to as a deep cleaning. The area is numbed with local anesthesia for your comfort. Using fine hand instruments and an ultrasonic scaler, we remove plaque, tartar, and bacterial byproducts from the tooth roots deep below the gumline, then smooth the root surfaces so the gum tissue can reattach and the pocket can shrink. Treatment is typically completed over one or two visits, usually one half of the mouth per visit.
Four to six weeks after scaling and root planing, we re-probe the treated sites. Most patients show measurable improvement: pockets are shallower, bleeding is reduced, and the gum tissue is firmer and pinker. Sites that have not adequately resolved may need additional localized treatment. Once active disease is under control, you transition to a periodontal maintenance schedule — typically every three to four months rather than the standard six-month cleaning interval. Periodontal maintenance is more thorough than a routine prophylaxis: it includes re-probing to monitor pocket depths, cleaning of the periodontal pockets, and a full assessment of how the disease is responding. This shorter interval is what keeps bacteria below the level at which the disease flares back up. Periodontal disease is managed, not cured — consistent maintenance is the most important part of long-term success.
Twice-daily brushing with a soft-bristled brush, daily flossing or use of interdental brushes, and — when recommended — a water flosser or antimicrobial rinse significantly slow bacterial re-colonization between visits.
